The Personal Background form gives your counselor an overview of several different aspects of your life that may be connected to the issue that brings you to counseling. If you have any questions or concerns about filling out this form, please let your counselor know. (If you are filling out this form for your son or daughter, please give their counselor your perspective of their situation, understanding that not all questions will apply.)
